-Perform reinsurance notifications
-Identify and report sentinel events
-Variance from care concerns
-Discharge planning
-Care needs of inmates/patients.
5. Actively engaged in discharge/community re-entry process for those inmates requiring medical follow-up.
Requirements Include:
1. Graduation from an accredited School of Nursing and current nursing licensure. RN licensure preferred
2. Minimum 1 year experience of care delivery in a managed care environment
3. Understanding of care delivery in a correctional environment preferred
4. Previous experience working with care management (CM). Chronic care experience highly preferred
5. Ability to work independently with general supervision. Must be detail oriented, multi-tasked, and possess exceptional organizational and follow-through skills.
6. Eligible for security clearance
7. Computer data entry experience
8. Skilled in MS Word, Excel and Power Point
